Active vibration reduction of plates - modal analysis

Hello I am currently trying to create model in ANSYS (ver. 13.0), and i have problem how setup properties contact actuator PZT to plate. PZT and plate must be connected by glue (epoxy).
Can anyone help me (step by step) how create correct model and doing analysis?
