Software to scan codes and create cross references

Good day everyone,

I was wondering if any of you know of a software that can scan codes and automatically create cross-references?
For example, clicking on 4.3.2.1 in a sentence would bring you to that section in the document.

Hyperlinks in .pdf documents accomplish this. Presumably, if you Adobe Acrobat you can create them. Never done it myself, though. I think there's a version of the AASHTO Bridge Design spec. with the hyperlinks already embedded.
