I had the same question as the original thread, which is not closed. I found some helpful advice and wanted to pass it on.

I created the following set on notes/specs that I use on my plans. As always, these notes are just for information - anyone using them does so at their own risk:
BONDED CONCRETE TOPPING INSTALLATION AND TESTING NOTES
1. CONCRETE TOPPINGS SHALL BE STRUCTURALLY BONDED TO THE CONCRETE PLANK BASE SLAB AND SHALL BE LIGHTWEIGHT WITH A COMPRESSIVE STRENGTH OF 4000 PSI AT 28 DAYS AND A MAXIMUM WATER-CEMENT RATIO OF 0.42.
2. THE BASE SLAB SHALL BE EVALUATED AND THE SURFACES PREPARED IN ACCORDANCE WITH ACI 503.6R CHAPTERS 2, 3 AND 4 AND THE INTERNATIONAL CONCRETE REPAIR INSTITUTE ICRI GUIDELINE NO. 310.2R-2013. THE BASE SLAB SURFACE PROFILE SHALL CONFORM TO CONCRETE SURFACE PROFILE (CSP) CSP-7 TO 9 AS DEFINED BY ICRI GUIDELINE NO. 310.2R-2013.
3. FOLLOWING BASE SLAB PREPARATION BY ROUGHENING AND CLEANING, THE BASE SLAB SURFACE SHALL BE PRE-TREATED BY WETTING OR APPLICATION OF A BONDING AGENT:
a. WHERE A BONDING AGENT IS NOT USED, SURFACES TO RECEIVE TOPPINGS SHALL BE PRE-WETTED TO A SATURATED SURFACE DRY (SSD) CONDITION BY SATURATING THE SURFACE, REMOVING ANY STANDING PUDDLES AND COVERING WITH PLASTIC SHEETING 1-DAY (NOT LESS THAN 12 HOURS, OR MORE THAN 24 HOURS) BEFORE PLACEMENT OF THE TOPPING. SATURATED CONCRETE SHALL NOT BE ALLOWED TO FREEZE.
b. WHERE ADHESIVE BONDING AGENTS ARE USED, THE BONDING AGENT SHALL COMPLY WITH ACI 503.6R CHAPTER 5 FOR EPOXIES AND CHAPTER 6 FOR LATEX AGENTS. BONDING AGENTS SHALL BE USED IN STRICT COMPLIANCE WITH THE MFR's WRITTEN INSTRUCTIONS WITH CONSIDERATION FOR WORKSITE CONDITIONS.
4. FRESH TOPPINGS SHALL BE AGGRESSIVELY WORKED ONTO THE BASE SLAB BY BRUSHING, VIBRATORY SCREED OR OTHER METHOD THAT WILL ENSURE FULL CONTACT WITH THE PREPARED BASE SLAB. TOPPINGS SHALL BE TESTED FOR TENSILE BOND STRENGTH IN ACCORDANCE WITH ICRI GUIDELINE NO. 210.3 AND ASTM C1583.
5. THE CONCRETE TOPPING SHALL BE FINISHED BY STEEL TROWELING TO OBTAIN A UNIFORM, LEVEL SURFACE. WET-CURE FOR A MINIMUM OF 7-DAYS AFTER FINISHING.
6. THE CONTRACTOR SHALL SUBMIT A TOPPING INSTALLATION PROCEDURE THAT COVERS THE FULL SCOPE OF THE TOPPING INSTALLATION INCLUDING BUT NOT LIMITED TO THE BASE SLAB SURFACE PREPARATION, MATERIALS AND EQUIPMENT AND THE PRODUCTS AND MATERIALS TO BE USED.
7. THE CONTRACTOR SHALL HAVE A MIN. 5-YEARS EXPERIENCE WITH THE INSTALLATION OF BONDED, STRUCTURAL TOPPING SLABS, REPAIRS OR OVERLAYS OF SIMILAR SCOPE AND NATURE TO THIS PROJECT. ALONG WITH THE INSTALLATION PROCEDURE, SUBMIT A LIST OF PROJECTS ALONG WITH DESCRIPTIONS OF THE SCOPE OF WORK, SQUARE FOOTAGE OF TOPPING INSTALLED AND OTHER RELEVANT DATA FOR EACH PROJECT.
8. IN ORDER TO DETERMINE THE BEST CHOICE OF PRE-TREATMENT FOR THE BASE SLAB AS DESCRIBED ABOVE, MOCK-UP TEST SPECIMENS SHALL BE MADE FOR THE FOLLOWING CASES:
a. EPOXY AND/OR LATEX BONDING AGENT.
b. PRE-WETTED WITH NO BONDING AGENT (SSD BASE).
c. DRY BASE SLAB OTHERWISE PREPARED AND CLEANED (i.e. NO BONDING AGENT AND NO PRE-WETTING).
9. A MIN. OF THREE (3) SPECIMENS FOR EACH ALTERNATIVE SHALL BE MADE AND TESTED FOR COMPLIANCE WITH ACI 503.6R, CHAPTER 4. THE TEST SPECIMENS SHALL BE APPROPRIATELY REPRESENTATIVE FOR SCALING TO THE LARGER TOPPING INSTALLATION AS NOTED IN ACI 503R, APPENDIX A.
10. PULL-TESTS FOR TENSILE BOND STRENGTH OF THE FINISHED, INSTALLED TOPPING SLABS SHALL BE PERFORMED AT A MIN. OF FIVE (5) RANDOM LOCATIONS ON EACH FLOOR. LOCATE AND AVOID TOPPING REINFORCEMENT PRIOR TO TESTING.
