Adding nozzle to bolted cover on App 2 flange

I have a small vessel with an Appendix 2 body flange and bolted cover. When adding a nozzle I select "From end of nozzle", but "Bolted Cover" up. In the Component Tree there is no separate listing for a bolted cover under the body flange as would normally occur when adding a blind flange to a nozzle.

How can I add a nozzle in this situation?
