Adding a new sheet after a sheet has been deleted.

Our drawing guru is out of the office today! I am incorporating an ECO that calls for a new sheet to be added to an already-released drawing. During the last revision, Sheet 3 was deleted - we maintain revisions using the Sheet Level Method.

It's not clear from ASME Y1400 whether the new sheet replaces the previously-deleted sheet. From my reading, it seems like it would NOT replace the deleted sheet - In that case, would the sht 1 title block say say "SHEET 1 of 3" while the new sht 4 title block says "SHEET 4"? Or am I wrong and the new sheet replaces the deleted one? Something else entirely?

Sheet one would say 1 of 3, and the new sheet would be sheet 3, with the revsion stating "THIS SHEET ADDED". It may be a good idea to state "NO RELATION TO SHEET 3 FROM REV X", but that may confuse more people than it helps.

In my previous days of military drawings, the deleted sheet would stay deleted, the new sheet added. Some customers would allow the previous deleted sheet to be replaced by the new.
You may also use "Supersedes previous deleted sheet", or something like that.

When I see a sheet that says "Sheet 2 of 4", I expect four sheets. I might not understand your internal process.

How do you explain why Sheet_3 is missing?

Suggestion: If there is no internal or external standard that gives clear direction, do whatever seems best, but add a clear, fully worded note on the drawing (preferably) or on the ECO, explaining fully what transpired.
 
We're using ASME Y.1400 drawing standards and most of our work is for the US government or prime contractors.

Our drawing guru came in this afternoon and I got an adequate answer about our internal standards, but I'm still curious about what other companies do in this case, especially if they are also government contractors.
 
A 3 sheet drawing has 3 sheets. Kind of hard to explain that they are numbered 1, 2 & 4.
 
Nevertheless that seems to be the ASME drawing standard - if a sheet is deleted the remaining sheets are not re-numbered. Adding the next sheet is where it gets tricky.
 
The deleted sheet will exist, but deleted. A note may be on the sheet indicating "deleted" or "removed", or "This page/sheet is intentionally left blank", or ?

(1) Contractor option
(2) Optional methods
(a) Renumber all affected remaining sheets
(b) Affected remaining sheets not renumbered (revision status of sheets block is updated with notations such as CANC or DEL)
