I need to replace a revese switch(contacter) on a 1/4hp single phase motor. The switch is a 6 terminal Square D. Could anyone tell me the correct wiring sequence?

It depends on the motor wiring scheme. There are generalities, but to be accurate you need to look at the motor's manual or see if there is a diagram on the inside of the connection box.

No manual & no diagram. Just a description saying to reverse motor interchange red and black leads, motor must be grounded.

Is this switch a two position "forward/reverse" switch or a three position "forward/off/reverse" switch.

3 position switch with 6 terminals plus ground. the motor has 4 wires, red, black, yellow(marked lead), green(marked lead ground).

red wire and black wire are attached to their respective terminals. yellow wire and blue wire(not green)come from inside armature. can you point me in the right direction. i've found a wiring diagram for the switch but i'm not sure which wires need to be attached to which terminals as i'm not sure whether this is a 4 lead or 3 lead induction repulsion motor. this is undoubtedly a stupid question but are one of these wires not a ground?

Repulsion-Induction, that takes me back. The few that I worked on were reversed by shifting the brushes. Got a picture?
"this is undoubtedly a stupid question but are one of these wires not a ground?" Possibly several of then are not a ground.
Got a picture? WE may be becoming confused. What does the nameplate say?

Name plate says Dayton Gearmotor Model 5K940, F/L RPM 60, HP 1/4, Torque 200 in/lbs, Ratio 29:1, Volts 115, Hz 60, Amps 4.6, Ph 1, Code R, S.F. 1.0, Amb 40 C. Cont, Class A Ins. Motor is minimum 37 years old it was put into use here around 1972 and was probably secondhand at the time. Attached is photo of inspection hole of motor. I am unable to get a clear photo of the nameplate.
