Phase to phase to ground fault is possible especially in high impedance grounding region, because strong possibility of healthy phase over voltage due to other phase ground fault. therefor as per IEEE 665 grounding connection and ground grid should be capable for huge fault current of double phase to ground fault currents.
 
I agree with you about phase-to ground-to phase as required ground fault current for grounding calculations.
However, IEEE Std 665 /1995 is withdrawn. It is based on IEEE Std 80-1986.
The last edition of IEEE 80 it is from 2013.
