Test Sheet For Anti Static Flooring

I'm looking for a typical test sheet used for recording test results for anti static flooring, any help would be much appreciated,

Are you going to record attributes (pass/fail) or variables (value measurements) data results?

Unless I had a specific requirement for one or the other, I would probably modify an existing form such as the one for testing ESD straps, assign it a new form number for revision control & get it into the QMS (Quality Management System) document control system...
