TEMA has a equation for calculating the maximum recommended spacing between bolts based in part on the flange thickness. It is a little ambigious to me if it is refereing to the flange material thickness or the width of the flange that the bolt is normal to?

Any thoughts?

t is defined by the flange thickness at the edge. If you were using a 24" 150# B16.5 flange, the value of t would be 1.875".

I.E. one does not include the value of any raised faces or hubs.

As an editorial, I can't remember the last time I designed a flange where the actual B on my flange came close the value of Bmax. It usually results in excessively thick flanges.
