Stopple Tee

Has anyone encountered a problem where the Stopple became stuck in the pipe and was not able to be removed? If so what options were used to remove the Stopple?

Did you ask the manufacturer for advice (I assume you are using a name-brand Stopple hot-tapping setup, e.g like in the link below?)
