SRU FLAME SCANNER ISSUE

There has been a flame scanner failure in our sulphur recovery unit where it was found that the flame scanner port assembly was red hot with the scanner face plate compromised and a coupling had melted resulting in the flame scanner suspended by a conduit. what could have led to this type of failure from a process standpoint or maybe refractory and orifice issue.what would be the likely cause?
 
Loss of purge gas, loss of cooling, poor installation, poor design of mounting.

Thanks..can you throw more light on how the loss of purge gas could cause this? we use Nitrogen to purge
 
trace the tubing/pipe back to the nitrogen source and investigate.
 
Was there positive indication of purge gas flow to the scanner before this failure?
 
The typical nitrogen purge system to the scanner is equipped with block valve, regulator, float meter, etc. May verify if any blockage in the N2 purge system.
