Single port submerged diffuser flow velocity

I need to solve a problem that involves shunt flow through a single port in a submerged pipe.

The problem concerns blood flow through a tube with a soft-edged orifice, but any help on the submerged single-port diffuser would provide invaluable insights.

I know the diameter of the pipe, the port diameter, the pressure at two points up- and downstream of the port, and the pressure of the fluid in which the pipe is submerged.

I need to find the rate of flow through the orifice.

I've investigated Bernoulli's equation applied to branching flows and branch angle correction factors but would welcome a brief tutorial explanation of the fluid dynamics of this configuration, or a pointer to a relevant exposition whether online or in a textbook.
