references on root cause analysis

Can you suggest any particularly good references that deal with root cause analysis? Most of my involvement is with transmission and distribution equipment used by electric utilities.

I know there's the references for the power engineer, but this is a little different. Actually many of the posts here deal with root cause analyses, so I'm sure there may be some nuggets out there.
 
Surely someone out there can suggest a decent book on root cause analysis!
 
"Electrical power equipment maintenance and testing" By Paul Gill, has a short section on root cause analysis. In general the "ask why five times" method works pretty well. Is there a more specific subject you are looking for?
