PSV SMALLER DIS. SIZE THAN INLET HELP

In any case is it possible PSV suction size is 12" and Discharge is 10".

The RD piping is 12", not the PSV inlet. See the reducers at PSV inlet and outlet. What is the inlet and outlet flanges sizes of the PSV?
