Pile supported strap footing

Does anyone have a good reference or an example of a pile supported strap footing? I am specifically looking for answers such as:
1. When should a pile supported strap footing be considered as compared to a combined footing specifically when the footing supports two columns?
2. What is the distance criteria between the two columns that is typically considered to decide on a strap footing vs combined footing?
3. Is a single pile a good option to consider under each of the column footings? If yes, how is the transverse stability considered when there are lateral loads in the transverse direction?
4. Is any contribution from soil considered when designing a pile supported strap footing?
5. If the pile to footing connection is a pin type connection, and if multiple piles are required under each footing to resist the applied loads, what kind of force distribution is considered in the longitudinal direction of the strap footing?
Any information regarding this topic would be appreciated very much.
