Can anyone tell me where I can find this item? Your help is greatly appreciated.

RJ45 is 8P8C (often used for Ethernet), and RJ11 is 6P2P (often used for POTS). What's the application? If you're using Cat 5/6 wiring for POTS, most installers would simply reterminate. If you must have an adapter, you may have to make an adapter cable (with dual female couplers as required).
