internal connections for 3 phase motors.

ran into a problem last week trying to reconnect a 2 delta, 575v 3 phase motor to 4 wye 480v. does anyone know where i might obtain some internal winding connection diagrams for 3 phase "concentric" wound motors. have all the lap winding connections, but am lost when it come to concentric windings.
 
How many leads come out of the motor?
 
six leads out, u1,v1,w1,x1,y1,z1. two wires on a lead, 12 groups 24 ends, all starts are lines, all finishes are jumpers.
 
Do you know how to do the "spiral" diagram to label the leads 1 through 12?
Take one of the delta's and label them 1 to 12. Do the same with the other.
Your leads should be 1,2,3,10.11 & 12.
Jumpers are 4 & 7, 5 & 8 and 6 &9.
 

...12 groups 24 ends...? Aaaagghh! Please pardon my outburst. My head just exploded.
 
Talk about your head exploding... we just did a two speed two winding motor. 2/18 pole! The 2 pole comes out to 12 leads and the 18 pole works out to 108 leads!
 
Suggestion: It will be more accurate to perform the markup in terms of u1,v1,w1,x1,y1,z1 by adding appropriate suffixes.
