Installing thermal break at existing walk in freezer wall

Existing walk in freezer showing signs of thermal bridging at base of wall between freezer and dry storage area. Any ideas on how to install thermal break in existing slab on grade? Idea was to cut opening in slab 4" from freezer wall parallel to wall, install thermal break continuous from under slab insulation to 3" above slab , install 2 layers of 2" rigid insulation on 4" space between freezer wall an installed thermal break, then forming and pouring 4" concrete over above thus creating an approximate 6" x 6" insulated "curb" at base of wall panel. Anyone have any suggestions/prior experience similar?
