In Line Switch Failure

Hi,

I am looking for examples of in line disconnect switches (in-line being a switch with only a single insulator separating both ends of the switch when open) tracking across the single insulator when open. This could cause an isolated section of line to remain live with the switch open. Voltage is medium (15kV class).
