implementing codes/standards in a refinery 1

1. what methodology should a refinery follow to adopt codes & standards and 2. are there any elobarate reference documents available on this issue?. AIChE does give information but it is not elaborate enough 3. How to fix responsibility for implementing codes/standards and making them current internally as the function may belong to various departments?
 
Please be specific in your thread. There are various API and BS code for the various unit of operation, eg pumps have their own code of standard.
 
ISO's 9000 and 14000 will do a very nice job of laying out the elaborate procedures for all three of your questions. Also, the OSHA HAZWOPER,1910.120 - Hazardous waste operations and emergency response has a lot of mandatory procedures that are good for adapting to other programs.
