It is available for purchase at http://www.concrete.org and is being or has been revised to include some newer photographs and editorial changes. If you can get the old Richmond manuals, do so. Otherwise DAtyton_ Richhmond info can be accessed at http://www.daytonrichmond.com/product_category.cfm?catID=10&vtype=2 Ratay's "Handbook of Temporary Construction" dicuss not only forming but shoring reshoring and unshoring of formwork.
