Foreced Convection in a Heated Tube

I am looking for a formula to estimate the temperature rise of pressurized water in a heated pipe. The relevant information are:

Water pressure:10,000 psi or 69 MPa
Water flow rate: 0.1 gal/min or 0.378 liter/min.
S.S. High-pressure tube: wall thickness & ID = 2.12 mm.
tube length: 6 m
thermal conductivity = 21 W/mK.
Temperature of pipe wall 200 deg C.
Inlet water temperature 20 deg C.
Heat transfer coefficient of water: 500 - 1000 W/m2K.

What is the water temperature at the exit of the tube ????

No, this is an experiment I am setting up for a research project.
 
every basic text on heat transfer will have the equation for T(exit) in this situation.
especially look in the "heat exchanger" chapter under Log Mean Temperature Difference.
