I wonder in what world they figured a floor would support an excavator.

How did it get up there to begin with?

The sad truth is that it was probably never checked further than applying a uniform load over the equipment footprint. There are a lot of cowboy style contractors in the demolition business -- and a lot of contractors who happen to win a job that includes some demo on the side, and don't want to pay for an engineer.
