concrete cracks

On a new (4 months old slab) pre-stressed conrete slab, when is a crack a sign of structure failure on the slab and should need an special attention?

Was the cincrete slab pre-stressed in both directions?? Also, where did the crack occur? was it parallel to a line of prestreessing cable??

The concrete, I believe, was pre-stressed on both directions. The a crack occurs parallel to the cable and started from a corner and the other one from the edge and mid of the slab.
