Combined Stress on Weld

My question comes after talking to someone about the (CJP, for discussion) weld at the base of a round vertical pipe to a transverse base plate. (think light pole design).

I've seen this interaction equation published...somewhere...but I can't recall where:

Can anyone point me to a published reference for this equation?

Vu = effective weld stress per unit length (kips / inch)
d = pole / weld diameter
Fx = axial load, Fz,Fy = base shear
Mx = torsion, Mz,My = moments
Aw = weld area, Sw = weld section modulus, Jw = polar moment of inertia

I cannot find the publication of the formula you are looking for however this link shows how to calculate fillet welds for strength for all types of loading.

Looks about right to me, you should be able to derive it yourself fairly straightforwardly though for peace of mind. Assume the weldment remains elastic, and take the sum of squares of shears that are normal and parallel to the axis of the weld.
