Anchor rods in pedestal, cracked or uncracked concrete?

When using ACI318 App D for assessing the shear capacity of anchor rods in a pedestal can the concrete be assumed to be uncracked? i.e. ?c,v=1.4 (Clause D.6.2.7)

Clause 7.10.5.6 and R7.10.5.6 seems to suggest that cracking could occur in which case ?c,v=1.0 ?

Can someone give me examples of the reinforcement required for the three cases for cracked concrete given in D.6.2.7 ?

Admittedly I am not a frequent user of ACI-318 however for the example that you described I would be inclined to go with the cracked concrete value of ?c,v=1.0. Unless you have a No.4 tie which is preventing the breakout of the anchor in which case increasing the breakout strength of the anchor by 20% (?c,v=1.2).

I am visualizing your pedestal as a raised concrete plinth supporting a column which is subject to small/moderate shear and pull-out forces.
 
Asixth, that's my reading too. In my case I would specify a 12mm bar instead of #4 bar. I wonder if 2-#3(10mm) bars would work?
 
sdz... 2#3...0.22 in2 and 2-10M 0.32 in2... close?
