Airliner engine fire included part installed backwards

I'm trying to figure out the vibration part. The report says all of the fan blades were present. Maybe they haven't looked inside the engie for lost compressor or turbine blades. Maybe an incorrectly positioned stator could cause vibration. A fuel leak that was severe enough to cause compressor stall?
 
There's a lot of esoteric terminology in there, so I really don't understand most of it. I did recognize the safety wire installed backwards, can only serve to make things loosen up. If I understand correctly, the loose VSV actuator could cause a lot of inconsistent performance that might be diagnosed as vibration.
 
Lockwire is an art.

And if done incorrectly can cause things to self destruct.

Once had a hydraulic pump explode on a garrette engine due to it on the safety valve mount bolts. No fire thankfully, one helluva bang though.
 
It's really simple if you do it right. The problem i see most of the time is that people don't know that the direction of twist needs to reverse after each fastener. This ensures the wire coming through the hole is on top of the wire that wraps around.
