Can you share Apendix A5 document in AASHTO ?

Appendix A5 is a 3-page outline that tells you what sections of AASHTO to follow; apparently you have at least the first page (BTW, your post like the 6th Edition; there's a 7th edition in use as well). The Management does not like anyone posting copyrighted materials, if you're missing pages 2 and 3 I can't put up them up.

With that said, just Google LRFD Concrete Bridge Design, you'll find numerous examples prepared by DOT's & FHWA. Here's a link to the Caltrans spec:
