AASHTO fatigue - prestressing tendons - source of requirements?

Hello,

Does anyone know the source of the 10 ksi and 18 ksi allowable stress ranges for the fatigue limit state of prestressing tendons? The commentary cites no references. (This is in AASHTO LRFD BDS para 5.5.3.3.)

Also, are those limits applicable to both ASTM A416 strand tendons and ASTM A722 bar tendons, whether prestressed or post-tensioned, bonded or unbonded?

Finally, are there any good reference works for fatigue design of A722 bars and related couplers and nuts?
