Cold formed heads may be stress relieved separately to the vessel (prior to installation) if the completed vessel does not require PWHT.
Would this then be classed as fabrication heat treatment ?
A PQR never expires.
If WPS requirements are superceded by a new edition you rewrite your WPS.
If testing requirements are changed and become additional you perform those tests and add to your PQR.

As the Client is paying they can basically ask for anything they like that is additional to code requirements.
I just try to point out to the Client the actual code requirements may be totally different to a 30 year old...

My response to the OP was going to be two cycles is a Client requirement to cover initial PWHT and a possible second cycle if a repair is performed.

One flanged joint only - extend the pipe connected to the Exchanger.
Install the 10" (or 8") stub on the extended Exchanger pipe

Took a sample ( exact thickness and diameter) boiler tube and machined multiple slots at increasing depths on the internal surface.
Radiographed that tube as a reference graph.
After radiography of suspect tubes the density of any internal corrosion was compared with...
