I'm not sure if it's been shared on this forum before, but the UK has a reporting scheme for engineers to anonymously report safety issuesthey come across either through observations on site or projects they are involved in. It's to act as a share of knowledge more than anything, some articles are thought provoking so I thought it might be of interest to some.
