There was a previous post with much speculation on the structural cause of the sign falling off its column. Does anyone know the final answer? Were the bolts too small? Holes too big? Holes didn't line up in the two plates? Too thin washers rusted?

If there was a significant investigation into that event, I have not been able to find out about it.
