Multihead chemical injection pumps diaphragm failure

Dear All,
I would like to understand specific design safeguarding items about multi-head (dedicated head for each injection point) pump design:
a- does each head has its own diaphragm?
b- in case of failure of a diaphragm of one of the heads, would it be fair to initiate a shutdown of the complete pump (as part of the ESD logic). In other words, would a diapghram rupture in one of the heads cascade to the neighboring heads which mandate full shutdown of the package?
c- do you recommend initiating a shutdown in case of any head's diaphragm failure, or a high pressure alarm would suffice instead?

Note : It makes sense to shut down the pump in case of diaphragm failure .
