The one vault that I did ended up with two layers of reinforcing everywhere and some robust connections between the doors and the surrounding structure.

I like to debate structural engineering theory -- a lot. If I challenge you on something, know that I'm doing so because I respect your opinion enough to either change it or adopt it.

I've built a vault. I don't remember the sides or top, but the bottom was something like 3 layers of #8 on 8" centers.
It was pretty cool to build as when you were finished there were little one inch squares left where you could see through all 3 layers.

a little less than halfway down the page under Special Support Areas. This is what we use for National Guard arms vaults. Not sure what the requirement of bank vaults are though.
