I have scoured the interweb and this document does not appear to available. Does anyone have it, or offer directions for obtaining it?

I started with #3 on your list, which is a good summary. It appears the original report exists only on microfiche, but coule be tracked down with some effort. I might try sending a query directly to the original agency that issued the report.
