I have a Western Digital external hard drive that exits the sleep mode for no apparent reason when nobody is using the computer.  What can cause this?

Plenty of things... network traffic, OS decides it needs to empty the fast cache, some background process wants to install an update, etc.

If you installed the Smartware that comes with the drive, it does things in the background when your system is idle.

Will the external hard drive go into the sleep mode if the Smartware is not installed?

I got frustrated with the software's CPU usage so I uninstalled it. If I recall there may be a help file associated with it if you are still using it. I just manually back up things and move files, then shut it off when I'm not using it.
