Weird Surface Defect (Honeycomb like)

Yup, looks like mechanical damage to me, nothing related to the welding itself.
It could have been there to start with, and you reveled it cleaning up the weld.
But more likely it is either from your weld prep or your weld cleaning.

Most definitely chipping hammer....I bitch about this daily.
 
If its a powder it could be "bridge breaking" by an operator?

We too suspecting the defect is caused by chipping hammer.

Material had been PMI tested. All chemical composition is accepted as per code.
