Or sometimes flat part of pipe is in middle of pipe. Is there easy way to design such thing?

I think I know what you mean, but could you provide at least a picture so as to confirm exactly what you're looking for?

1. For the model, to get a rough but functional geometry, I'd use a Ruled Surface between a circle and a rectangle.
2. Then create a User Defined Feature (see attached) that you would insert at the end of your pipe on demand and be able to choose which of the two possibilities you want (offset or not).

For exhaust pipes, there are sometimes "flats" which are pressed into one side of the pipe in order to fully clear other components or to provide access to components that are above the pipe. We routinely model the pipe up to where the deformation starts and after the flat area, then model the flat area, then create Through Curve Mesh bewteen the pipe and flat (similar to above but with a bit more control of the transition). For our process, this area will typically be natural flow of the material since the shape in that transition area isn't goint to be the same every time.
