Waiving a Hydrotest

We are planning on waiving a hydrotest by meeting the requirements in API 653.

One of those requirements is written acceptance by the owner/operator. Is there a standard form somewhere in API 653/650 or 579 we can use? I can't seem to find one...

There is not. Just write up a short memo, with signature blocks for the Inspector and the Owner, and possibly the Engineer.
