Utility hole through existing end bent

I am currently designing a widening/rehab on a historic concrete bridge. I need to place two utility sleeves through the end bent. I plan on having the contractor core through the concrete (12" dia and 14" dia sleeves). the holes are located approx 5 1/2 feet below the deck surface.

I am planning on detailing the pipes as a cored hole 2" larger than the diameter of the pipe and non shrink grout around the exterior of the pipe.

I am contiplating placing some #4's to prevent any cracking, but will not have alot of clearance....

Any suggestions?

If it is non-shrink grout, do you actually need much anti-cracking steel? It looks like you'll have a big heat-sink around a small amount of grout. Is there significant structural loading being applied to your holes?
