use of saddle washer to fasten through shaft

I am trying to locate a saddle washer to provide a flat surface for a 1/4 inch SHCS mounted transversely through a 5/8 diameter shaft. I need a source for the sadle washer and the best solution to lock the fasteners. I am currently using a flat washer and safety-wire.
