How do you take into account the bearing capacity against the bell when calculating the uplift capacity of a belled caisson? Can this component of resistance be combined with the frictional resistance along the shaft?

Only if the rebar cage extends into the bell.

Well...you do get some resistance from the shear strength of the concrete at the bell/shaft interface also.
