Triple Headwall

I have a project that I have 3 54-inch pipes for a commercial driveway. I am having trouble finding a triple pipe headwall that will accomodate 3 54-inch pipes. Anyone got a detail for this...I know it will have to be poured in place.

Every states' DOT's have standard plates for headwalls with every possible pipe size, wing wall alignment and number of pipes.

Space the pipes at 1.5 times their diameter and use the same cross section headwall you would use for (1)( 54" pipe).
