There is a comprehensive EC Directive covering the Design and Approval of Tow balls and Tow Couplings including the mandatory test requirements for such a system and how it must be marked.
I would imagine there is similar legislation in the US.
I sold around 25 fatigue test systems when the legislation regarding testing was introduced inn the Nineties.
