Either the add-in is corrupt or if you have more than one person on your network changing the toolbox they have screwed up the TB part files in some fashion. I always protected the TB files from being changed by others and added the sizes needed when someone had a special request.

I thought there was an online tutorial, but I didn't find one.
You have to have it setup on the network and then you can create a password to protect the DB and you can set the options as you need it.
