tethered structural washers

I'm looking for washers that have a tab and hole similar to the one shown in the attached picture of ordnance fastened with structural bolts used with tethered washers. They need to be structural and anti-corrosion coated.

Once I source the washer, I need to determine a method of keeping the bolt and washer from separating, while still allowing free rotation.

Thanks for the help
 
Has anyone ever used electrical ring terminal connectors for this purpose? If only I could find one made of aluminum, not nickel plated copper.
