TC40/41 Flaking

We had some of our 17-4 PH Stainless Steel recently coated with TC40 and TC 41 films. The coating was fine when we received it, but it started to flake as we started passing another metal along it (it's used as a clamping jaw in a feeder system with metal clips). The 17-4 was solution annealed before being coated.

The question is, what could have gone wrong in the process? Also, what can be done to fix it?

The 17-4 is coated to lower friction as well as increase the surface hardness.
