I'm looking for a checklist for use in performing an on-site structural evaluation of an existing building.

Check out SEI/ASCE 11-99, "Guideline for Structural Assessment of Existing Buildings".

You can order it directly from ASCE Publications for $65 ($48.75, if you are a member)
