Specification of 12 X True Postion

The inspection report shows the specification requirement:
Feature: Position
Specification: 12 X .125 +.002/.000 [(TruPos symbol) dia.002 (M) | A | B ]

When I perform the CMM measurement, I am not sure about the requirement of this callout:
All three holes (callout is actually a True position of the PCD of all holes)?
or individual hole True Position?
And could it be the same thing?

Each one of the 12 each .125 +.002/-.000 holes are to be checked for location to TP Dia. .002 (M = Maximum Material Condition (this provides bonus tolerance to the TP Dia. tolerance) with part aligned to DATUMS A & B.
I am not sure what you mean by "all three holes" or "PCD".
The CMM report would show the XYZ nominal values, the actual values and the deviation amount.
The Inspection Report could be filled out by attaching the CMM report or, showing the range of all 12 ea of the holes, I.E.: TP DIA. .002 checks .0015 to .0019

Yes, and don't try to cram all 12 values in one space on the inspection report! Don't laugh, I've received inspection reports for dimensions that were 2 X or 3 X dimensions and they wrote them all in one space because when they laid out the inspection report they didn't allow for the multiple readings.

What I mean by PCD is Pitch Circle Diameter.
12 X, that is a lot of measurement!
