I have an old drawing that has a table on it. One of the columns in the table is for a radius dimension. To the right of the dimensions in the table there is either one Plus sign (+), two plus signs (++), or three plus signs (+++). Has anyone ever seen this type of tolerancing, if in fact that is what it is?

maybe....
the +, ++, +++ is referring to 3 notes somewhere in the drawing.

Quite possible. Also, may be order of preference ("first choice", "second choice", etc.)

But either way, it has to be explained somewhere on the drawing, as it doesn't seem to be part of any drafting standard.

We rehabilitate hydro turbines that were built over 100 years ago. Many of the customers still have OEM daring of the equipment! I have seen the "+" plus' on these old drawings. We interpreted them to be a surface texture symbol. More + finer the finish. Maybe this appliers to your "old" drawing too.
